MediM^TM for improving medication adherence and persistence

MediM is designed to increase a patient's adherence and persistence to their medication therapy. In a landmark study conducted by Mt. Sinai Medical Center with Pediatric Liver Transplant patients the MediM solution demonstrated significant improvement in patients outcomes. The results were published in the 2009 issue of Pediatrics.

DiabeText^TM for managing Type 1 and 2 Diabetes

DiabeText was originally designed specifically for patients suffering from Type 1 diabetes, however it quickly became evident that it provides significant value for Type 2 diabetics too.

DiabeText is an “Intelligent Logbook” designed to keep track of glucose, carbohydrates and insulin, help patients remember to take their medications on time and as prescribed, provides them with nutritional tips and physical activity tracker and tips. All recorded data is charted on an on-line account, can be forwarded to the provider.

Kortxt^TM - for managing mood disorders

Kortxt was designed for patients with mood disorders to help them take their medications as prescribed and to track their moods. Using the mood tracker patients and the health-care team can determine impact of medication and look for correlates in dosing.

An exception notification algorithm detect episodes of low or no compliance, and undesired mood trends upon which it notifies health-care team via e-mail or text message. Depending on the severity of the undesired trend notifications can be sent out immediately or on a periodic basis (e.g. weekly).

TransitionTexts - designed to help patient transition from in- to out-patient setting

The transition between the inpatient and outpatient setting is a high-risk period for patients. TransitionText provides patients and their caregivers with relevant and timely information post hospitalization, and captures information back from patient via biometric tracking and surveys to ensure any undesired symptoms are captured early. Care management team is notified when patient adherence and biometric data values and / or trends are of concern.

Welltexts^TM - for helping individuals keep physically active, and prepare and eat nutritious meals

Welltexts are designed for people who are either suffering from a chronic illness or at risk for developing a chronic illness such as diabetes or hypertension. The solution consists of a motivational and educational messages about healthy living. Participants are also prompted to be more physically active and can report and track their activity levels.

Welltexts programs are usually used by employers who want to provide a wellness service as a value-added benefit to their employees.

CareSpeak's modular designed allows for a quick design and build of a customized solution for any customer. CareSpeak's mobile Health manager consists of the following modules that can be used individually or in any combination. Our professional services team works closely with customers to design the right combination of modules and develop the appropriate content.

Medication

Ensures patients take medication on time and as prescribed. Covers a variety of dosing regimens and refill alerts. Allows for caregiver escalation alerts in case of a missed intake and for refill reminders.

Education

Consist of campaigns of messages designed to educate patients on a variety of topics from "tips" on living with a specific illness, and medication warning signs / side effects, to nutritional advice.

Motivation

Designed to ensure patients know how how they are doing versus their health goal or peers suffering from same illness. Messages can go to patients and / or caregivers who can get tips on how to motivate patient.

Biometrics

Enables patients to report and track biometrics that are vital to know and understand in order to manage their illness better (e.g. blood sugar for diabetes, weight for CHF, etc.). Allows for caregiver escalation alerts if reported values out-of-target range.

Appointments

Ensures that patients keep their scheduled appointments resulting in reduced no-shows. Provider staff is notified of responses. Unique feature checks post-appointment if patients actually attended appointment and notifies staff.

Incentives

Provides the option to give patients rewards and additional motivation to keep up with their health goals. Patients can be rewarded via refillable co-pay or debit cards. Debit card refills are subject to an independent third-party (e.g. clinician) validation.

Surveys / Quiz

Enables program managers to collect information and data from patients in a 1-to-1 or hierarchical questions-answer format. Other modules can be activated based on Survey responses. Quiz format can be used for a more interactive experience.

Protocols

Helps patients keep up with specially timed protocols that differ depending on treatment. Patient and / or Caregiver is notified days, weeks, or month leading to- or from- a medical event with important medical information and / or instructions.
